Your First Appointment as a New Patient
Please plan to arrive 15 minutes prior to your first appointment in order to fill out the medical history and insurance forms and other pertinent information. We request you bring the following items with you:
- All pertinent information regarding your insurance if applicable.
- The names of any medications you are currently taking. Medications affect your health and they may influence dental treatment.
- Information regarding medications you may require prior to any dental procedures. History of heart valve damage, joint replacement, or rheumatic fever may indicate the need for antibiotic pre-medication.
- Your work phone number and name of your current physician.
- Current x-rays if they have been taken within the last year.
With this information in your chart, our dental team will perform a complete dental head to neck examination and take any x-rays pertinent to your dental health. A treatment plan will be formulated by Dr. Chiang that is tailored to your specific needs.
